What Is Anesthesia Day?

Every October 13th is "Anesthesia Day" (麻酔の日, Masui no Hi) in Japan. On October 13, 1804, Japanese surgeon Hanaoka Seishū (華岡青洲, 1760–1835) performed the world's first successful surgery under general anesthesia — a mastectomy for breast cancer — on a patient named Kan Aiya in Wakayama Prefecture. Hanaoka's achievement, accomplished using a botanical anesthetic compound he had developed through years of research, predated Western discovery of ether anesthesia by 42 years and represents one of the most significant medical achievements in Japanese history.

Hanaoka Seishū's Breakthrough

Hanaoka Seishū spent approximately 20 years developing his anesthetic compound — "Mafutsusan" (麻沸散, also spelled Mafeisan) — a mixture of extracts from plants including datura (Datura metel, チョウセンアサガオ), aconite, Angelica, and other botanicals. The compound, administered orally, induced a state of unconsciousness sufficient to permit major surgery. Hanaoka tested formulations on animals, then on human volunteers — including his own wife Kae (加恵), whose eyes were permanently damaged during the testing process, a sacrifice documented in the novelist Sawako Ariyoshi's famous 1966 novel "The Doctor's Wife" (恍惚の人/ 華岡青洲の妻).

The First Anesthesia-Enabled Surgery

The October 13, 1804 mastectomy required the patient to remain unconscious for an extended period while Hanaoka operated — a procedure that would have been impossible without effective anesthesia, as the pain would have caused the patient to move or die of shock. Hanaoka's success demonstrated that planned surgical intervention in the body's deep structures was possible if the patient could be rendered insensible to pain — a concept that transformed surgery from a desperate last resort into a planned therapeutic procedure.

Anesthesia's Global Impact

Although Hanaoka's work was not widely known outside Japan during his lifetime, his achievement stands as one of medicine's most important milestones. When Western anesthesia (ether) was developed in the 1840s and chloroform shortly after, the transformation of surgery was immediate and dramatic — enabling procedures that had been impossible while patients remained conscious. Modern anesthesia, combining intravenous agents, inhaled gases, and local nerve blocks, makes virtually any surgical procedure safe and painless.
